Therapy Apps Like BetterHelp vs. a Local Therapist in Lisbon
Apps lowered the barrier to entry for millions of people, and that’s a genuine good. Here’s what actually differs if you’re choosing between one and an independent local practice.
Platforms like BetterHelp made starting therapy faster and less intimidating for a lot of people who otherwise wouldn’t have tried it. That’s worth saying plainly before getting into the differences — this isn’t a takedown, it’s a comparison of two genuinely different models.
How you end up with a therapist
On an app, you typically fill in a questionnaire and are algorithmically matched to a therapist from a large pool, with an option to switch if it doesn’t feel right. With an independent practice, you choose based on a bio, credentials, and modality before your first session — you’re picking a specific clinician, not entering a queue.
| Therapy app | Independent local practice | |
|---|---|---|
| How you’re matched | Algorithm, based on a questionnaire | You choose based on bio/credentials |
| Modality choice | Usually general/unspecified | Specific (e.g. ISTDP, EFT) stated upfront |
| Continuity | Can vary if a therapist leaves the platform | Same clinician for the duration of work |
| Format | Chat, video, or both, subscription-based | Scheduled video or in-person sessions |
| Local knowledge | Often not location-specific | Knows local insurance, SNS, referral options |
A data privacy episode worth knowing about
In March 2023, the U.S. Federal Trade Commission reached a $7.8 million settlement with BetterHelp over sharing sensitive health information — including answers to intake questionnaires — with Facebook, Snapchat, Pinterest, and Criteo for advertising purposes between 2017 and 2020, despite privacy assurances to users. It was the first FTC settlement of its kind to require direct refunds to affected consumers. BetterHelp did not admit wrongdoing as part of the settlement.
This doesn’t mean every therapy app has the same practices, but it’s a concrete, documented example of why it’s worth reading a platform’s actual data policy rather than assuming “healthcare” automatically means the same privacy standard as a doctor’s office.
Cost and what it buys you
App subscriptions are often cheaper on paper than paying per session for an independent therapist. What that price difference usually reflects is volume and standardization versus a smaller caseload and a specific, chosen approach. Neither is objectively “better” — it depends on whether you’re looking for low-friction, general support, or a specific method worked with a specific person over time.
If you’re in Lisbon specifically
One thing a general app can’t offer is local context: how Portugal’s SNS system works, whether a government therapy voucher applies to your situation, which private insurers are common with international employers here, or a warm handoff to in-person sessions if you’re ever in the city. Our own guide to finding a therapist in Lisbon covers those specifics if that’s what you’re after.
Common Questions
Is BetterHelp bad?
Not inherently — it’s a legitimate option that works well for many people, particularly as a low-friction way to start. It’s a different model from choosing an independent clinician, with different tradeoffs, not a “worse” one across the board.
Can I get ISTDP or EFT couples therapy through an app?
Generally no — most general therapy apps don’t let you select a specific evidence-based modality upfront the way an independent practice does.
Do local therapists in Lisbon offer online sessions too?
Yes — our own practice offers online sessions across Portugal and Europe, alongside in-person sessions in Lisbon.
